  EISOD - FMEA Analysis Module
Workflow FMEA
The module applies the FMEA (Failure Mode and Effect Analysis) method for the analysis of failures occurred on the considered product or process, their risk evaluation of their and proposal and execution of measures for improvement. The FMEA Analysis Module proceeds from the scope of assignment from individual parts of the products. The potential failures of process are specified and are analogically step-by-step analysed. The main aspect is improvement of the product construction qualities and increase in operational reliability, but by the means of FMEA it is possible to reach also constructional improvements. The problem-solving team consists of the staff involved in the technology, logistics, production, quality (team members) and head of the problem-solving team (Moderator).
The FMEA Analysis enables as follows:
  • System FMEA of the construction
  • FMEA of the construction (design) ( K-FMEA)
  • System FMEA of the process
  • Procedural FMEA ( P-FMEA)

There are lists of all parts (which can be specified as per user within module’s codebooks) and product risks. Subsequently, all possible failures are described pursuant to the following aspects:
  • How could the function/mode of part fail?
  • In what way/mode could the part not match with the specifications?
  • What could a customer or a processor independently from the specifications consider as unsatisfactory?

Further steps of the analysis:
  • Risk rate/priority determination,
  • Decision on the order of realization of the remedial measures,
  • Implementation of the remedial measures,
  • Evaluation of the improved status.

Individual procedural steps with respect to the products are the starting points for the team research, procedural FMEA separately examines:
  • Potential failures,
  • Possible effects of these failures,
  • Possible causes of these failures.

It analyses and evaluates pursuant to prearranged coefficients (starting from 1 – insignificant, to 10 – very significant) three following factors:
  • Probability of the incidence of failure,
  • Significance of the causes of failure,
  • Failure detection probability.

Pursuant to the formula stated in the FMEA manual the following shall be calculated as product of three factors:
  • Risk rate – priority RR/P.

According to RR/P the following shall be determined and evaluated:
  • Remedial measures (RM) priority,
  • Remedial measures (RM) realisation,
  • Remedial measures (RM) efficiency.
